10th Annual Forum of the EU Strategy for the Baltic Sea Region
1.2. The EUSBSR Forum is held on 12th – 13th June 2019 at the Polish Baltic Frédéric Chopin Philharmonic in Gdańsk.
1.3. The EUSBSR Forum is organised by the Office of the Marshal of the Pomorskie Voivodeship based in Gdańsk (80-810), 21/27 Okopowa Street, in partnership with the Baltic Sea States Subregional Co-operation (BSSSC), in co-operation with the Ministry of Foreign Affairs (MSZ) and the European Commission, hereinafter referred to as the Organisers.
§2. Registration for the EUSBSR Forum attendance
2.1. In order to attend the EUSBSR Forum, each delegate shall register and choose conference seminars.
2.2. The registration for the event is voluntary.
2.3. The following is required to attend the EUSBSR Forum:
2.3.2. Having registered for the Forum, each delegate will be sent an e-mail confirming that the EUSBSR Forum registration form submission has been received;
2.3.3. The Office of the Marshall of the Pomorskie Voivodeship will verify the registration forms received;
2.3.4. Following the registration verification, till the end of registration process, each delegate will be e-mailed a QR code as the event admission ticket. The confirmation will be sent to the e-mail address provided in the registration form;
2.3.5. The delegates who have been rejected will be e-mailed a message informing them of the rejection.
2.4. The electronic registration form will be active from 1st April to 15th May 2019. The Organisers have the right to change the dates of the form availability.
2.5. The Organisers shall not be held responsible if a Delegate has provided incorrect or false data when registering for the EUSBSR Forum.
2.6 By submitting the registration form published on the EUSBSR Forum website, the registrant accepts the Policy provisions, as well as any applicable legal requirements or other arrangements that have been made.
§3. Personal data
Pursuant to art. 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, hereinafter referred to as GDPR) (OJ of the EU L 119 of 04.05.2016, page 1), please be informed that:

3.2. Your personal data will be processed based on Art. 6.1 c) and e) of GDPR, the Act of 11 July 2014 on the principles of implementation of the cohesion policy programmes, financed under the 2014-2020 financial perspective, the Act of 8 December 2017 on the State Security Service (Art. 21.6 and 7 in conjunction with Art. 3.1 and 2 and Art. 40), the Interreg Baltic Sea Region Programme 2014-2020, the Agreement between Investitionsbank Schleswig-Holstein, Germany and the Pomorskie Voivodeship for Project F004 Annual Forum 2019, for the purpose of your participation in the 10th Annual Forum of the EU Strategy for the Baltic Sea Region;
3.3. Your personal data will be disclosed to 2mgroup Komunikacja biznesowa Sp. z o.o. Sp. K. – the company we commissioned to process personal data in relation to the organisation of the Forum. Furthermore, your personal data will be disclosed to other entities who will be commissioned to provide services related to the processing of personal data (e.g. IT services providers). Such entities will be processing data based on an agreement with us and only as requested by us;
3.4. Your personal data will be stored until the end of the applicable archiving period;
3.5. You have the right to request from the data controller access to, rectification or erasure of your personal data or restriction of processing;
3.6. You have the right to lodge a complaint with the President of the Personal Data Protection Office;
3.7. The provision of your personal data by you is a prerequisite for your attendance of the event referred to above. You are required to provide these and your failure to provide your personal data will prevent the achievement of the above mentioned purpose of the event attendance.
§4. Final provisions
4.1. Each Delegate acknowledges that during the EUSBSR Forum photographs will be taken and that the Forum will be recorded. After the EUSBSR Forum, the photographs and the recordings can be used on the EUSBSR Forum website and in social media.
4.2. The Organisers shall not be held responsiblefor any belongings of the Delegates that might be lost, damaged or stolen during the EUSBSR Forum.
